By consenting to the Medical Massage, you give permission to receive the massage, understanding and knowing that therapeutic massage is not a substitute for traditional medical treatment or medications. You understand that the massage therapists do not diagnose illnesses or injuries or prescribe medications. You also understand that the risks associated with massage therapy include, but are not limited to superficial bruising, short-term muscle soreness and often exacerbation of undiscovered injury. You therefore release AAIE, AMMN and your massage therapist from all liability concerning these injuries that may occur during the massage session(s).
You understand the importance of obtaining clearance from your healthcare provider for massage and of informing the massage therapist of all medical conditions and medications you are taking, and to let the massage therapist know about any changes to these. You understand there may be additional risks based on your physical or emotional condition and that it is your responsibility to inform your massage therapist of any discomfort you may feel during the massage so she/he may adjust accordingly. You further understand & agree that that both you, or the massage therapist may terminate the session at any time, and that you have been given a chance to ask questions about the massage and your questions have been answered satisfactorily. You acknowledge that you have received a copy of the ‘Privacy Policy’, before the massage session begins.
In addition, you also understand and expressly agree that any claim you may have will be against the local massage therapist or the company she/he works for, and NOT against AAIE/AMMN or its officers, directors, employees, etc. . You further understand and agree with the following:

(AAIE) doing business as American Medical Massage Network (AMMN), the officers, directors, shareholders, employees, independent contractors, representatives & agents cannot be held responsible for any claim, action, loss, liabilities, expenses, costs, demands, etc. for any and all damages, directly, indirectly, and/or consequentially resulting or allegedly resulting from the conduct of, or services provided by any therapist and/or advertiser listed in this website.
American Medical Massage Network simply facilitates the availability of medical massage specialists. You agree that as a consumer, it is your responsibility to obtain the approval of your healthcare provider for this massage, check the laws of your state, the licensing requirements and/or practice eligibility, training & experience of the massage therapist, before agreeing to let her/him provide the massage or paying anyone or using services of anyone listed as a medical massage therapist, and/or specialist in the website.
In case of any dispute, or the dispute with anyone listed in this website as a massage therapist or an advertiser, may be resolved first by talking to her/him/them, informing AMMN of such a dispute in writing and if the same is not resolved amicably, by going to a local small claims court against such provider, failing which the dispute may be resolved by the American Arbitration Association, with a copy of such filing sent to American Medical Massage Network, enabling AMMN to consider canceling the Ad or membership of the provider in question. By using services of any provider listed in this website, you automatically and expressly agree that the laws of the State of Michigan, where AMMN/AAIE is incorporated, apply to this Agreement, without regard to principles of conflicts of laws or state where the service was provided or the provider / receiver is domiciled. You further agree that any lawsuits permitted under this Agreement (other than permissible claims in local small claims court) shall be brought exclusively in the Federal or State Courts located in the County of Macomb, State of Michigan, and you hereby irrevocably submit to the exclusive personal jurisdiction of such courts for such purpose.
