Terms, Conditions & Agreement:

Welcome to American Medical Massage Network (AMMN) organization. We appreciate the opportunity to explain the various terms, conditions and provisions that govern your affiliation / membership in AMMN, a special national network to market and promote your services as a qualified & licensed Massage Therapists.

American Medical Massage Network, thru this website, simply facilitates the availability of qualified medical massage specialists in various locations in every state. By agreeing to become an Affiliate / Member of AMMN, you consent to Agreeing to the current and future terms & conditions of AMMN, which are as follows:

1. You qualify & have met the ‘Criteria & Requirements for Membership’ as outlined in the Massage Therapy Membership Section in this website.
2. An Application for Membership has been completed, signed and sent to AMMN, together with the required documents.

3. You have completed the ‘Self-Evaluation Form for Level of Specialization’ and have sent, together with all supporting documents about your academic training, experience, CME/CE, etc. to AMMN, as per AMMN Guideline for Levels of Specialization.

4. Upon completion of all requirements for affiliation / membership, you will be so advised and a ‘Certificate of Specialization’ issued to you. You are welcome to display that Certification in your clinic. Although you will be in our database for the city/state where you are practicing, there is no listing as such in your state-pages. However, you are encouraged to avail the opportunity to have your practice listed in your state page Directory for just $10.00 per month – payable in advance for a year.

5. You accept that the AMMN reserves the right to contact & verify the information you have provided, as well as ask you to provide any additional information or explanation to authenticate the submitted information.

6. Because Medical Massage Specialist work with Hospitals, Clinics, Healthcare Providers and 3rd Party Payers who may require high credentials, AMMN has set very high standards to classify the ‘Level of Specialization’, you accept that AMMN will analyze & scrutinize your qualification, and may assign you with a different ‘Level of Specialization’, but that it is subject to review. You will be able to re-apply for higher Level of Specialization with additional qualification, experience, and/or recommendations every six months.

7. While there is no fee to join AMMN, we will deduct a 15% Administration Fee from the fees we will collect from the clients, which we refer to you for Massage. This fee is subject to change without notice.

8. Fraudulent or wrong information submitted to AMMN for your affiliation / membership and/or Level of Specialization is a cause of immediate cancelation & removal of your information from our database.

9. Consumers & Healthcare Providers will be regularly asked to evaluate your services, and any negative comments will be advised to you. AMMN will discuss all issues with you in writing and you will be allowed 10-days to respond for our re-evaluation. Failing to receive your timely response, and/or after three (3) negative reports, your affiliation / membership will be terminated, and you will be so advised.

10. The referrals received by the AMMN from hospital, clinics, physicians, chiropractors, physical therapists and other healthcare providers, as well as from insurance companies, self-insured corporations, or self-paying clients will be sent to you by email, allowing you to confirm one of the two dates the client requests.
You are expected to kindly confirm promptly, within 24-hours, one of those dates/times, or another date/time when you can accommodate the client.

11. Failing to confirm promptly will harm AMMN relationship with the referring health professional and consumer. After three (3) delinquencies, you will be given a warning, and if not improved you may not receive any further referrals.

12. If you state license is cancelled, for any reason whatsoever, or if you are charged for any misdemeanor or felony of any type, or in case of any unethical or immoral report about you and/or your staff member(s), your affiliation / membership will be terminated immediately. You are required to keep AMMN informed of any such violation.

13. Even though the clients receive our standard ‘Privacy Notice’ from our website, you are required to provide ‘Privacy Notice’ to each & every client. Failing to do so, or failing to comply with the HIPPA regulations, your affiliation / membership will be terminated immediately.

14. In addition to the required CE for licensing by your state, we highly recommend that you take AMMN courses, attend AMMN recommended CME or CE programs, Internship, etc. or equivalent programs offered by any other recognized organization, to upgrade your knowledge and skills in medical massage specialty. Please keep AMMN informed of any advance training you undertake.

15. You must inform AMMN immediately about any change in ownership, name of your clinic, address, tel. #, website, email, etc. failing to do so within 30-days of the change, will result in your affiliation / membership being suspended immediately. It can however be considered for approval once we have complete information about the change.

16. AMMN accepts copies of your license & other documents, but it may not investigate, review, validate, confirm or endorse the license, qualifications or abilities of any member offering services through AMMN Referral Program, nor does it guarantee or endorse the quality or efficacy of any services the members offer.

17. As a member, you are fully responsible for confirming your qualifications and for determining the appropriateness of any service or suitability of any particular massage modality. You are also fully responsible for any liability, lawsuit or damages occurring due to your negligence, action or inaction. You are also responsible to indemnify AMMN, its affiliates, officers, staff, etc. for your negligence, action or inaction.

18. The AMMN website contains certain images & contents, such as text, HTML, code, graphics, images, logos, buttons, icons, software and other material owned, operated, licensed or otherwise controlled by AMMN or other companies with which it has permission to utilize such services. Such website material is protected under the United States copyright, trademark and other laws. All website material, including AMMN logo is the exclusive property of the American Medical Massage Network (AMMN), and protected by U. S. and International copyright laws.

19. The AMMN authorizes you, as a member, to view and access a single copy of the website material solely for your use. Unauthorized use of the website material may violate copyright, trademark and other laws. You may not sell or modify the AMMN website material or reproduce, display, publicly perform, distribute, or otherwise use the website material in any way for any purpose.

20. The website and services may be used only for lawful purposes expressly permitted by this Agreement. You understand and agree that you may not use the website or services for any other purposes, other than as a massage therapist.

21. AMMN affiliates / members have limited rights to use the official logo of the American Medical Massage Network, on their business-cards, letter-head, envelopes or other promotional material. If you have your own logo, you may also use AMMN logo next to it. For all other use cases, specific permission must be requested in advance. Under no conditions may the AMMN logo be used for non-AMMN sanctioned events, publications or materials.

22. The AMMN website may contain links to third party sites. These links are provided solely as a convenience to you and not as an endorsement by AMMN of the contents on such 3rd party sites. AMMN is not responsible for the contents of linked 3rd party sites and does not make any representations regarding the contents or accuracy of material on such 3rd party sites. If you decide to access linked 3rd party sites, you do so at your own risk.

23. In the event that you have a dispute with one or more clients, patient, health providers or suppliers, you expressly release the American Medical Massage Network, and its parent – the American Academy of International Education Inc., its owners, directors, employees, etc. from any and all claims, demands, losses, damages, or other harm (under tort, contract, or any other basis; actual or consequential; direct or indirect) of every kind and nature, known & unknown, suspected and unsuspected, disclosed and undisclosed, arising out of or in any way connected with such disputes.

24. The AMMN does not warrant that the website will operate error-free or that the website and its server are free of computer viruses or other harmful mechanism. The website and services are provided on an ‘As Is’ basis without any warranties of any kind whatsoever. The AMMN, to the fullest extent permitted by law, disclaims all warranties, whether express or implied, including the warranty of merchantability, fitness for particular purpose and non-infringement, the AMMN makes no warranty about the accuracy, reliability, completeness or timeliness of the website, services of its members, software, text, graphics and links.

25. The AMMN’s maximum liability arising out of or in connection with the website, your use of the website or use of any services provided by AMMN or anyone listed herein, regardless of the cause of action (whether in contract, tort, breach of warranty or otherwise), will not exceed $10.00 (Ten dollars).

26. You, as an affiliate / member, agree to defend, indemnify, and hold harmless the American Medical Massage Network and/or its Parent – American Academy of International Education, Inc., a Michigan Corporation, its owners, directors, officers, employees and agents, from and against any claims, actions or demands, arising or resulting from your affiliation / membership with AMMN or services provided by you and/or your co-workers or employees, or breach of any of the terms of this Agreement.

27. AMMN reserves the right to make changes to our website, services, administrative fee structure, policies or this Agreement at any time, and without giving you any notice.

28. If any of these conditions shall be deemed invalid, void, or for any reason unenforceable, in any locality, that condition shall be deemed severable and shall not affect the validity and enforceability of any remaining conditions.

29. You agree that the laws of the State of Michigan, without regard to principles of conflicts of laws apply to this Agreement and any and all disputes or claims relating to your use of this website or your affiliation / membership or services. You agree that any lawsuits shall be brought exclusively in the Federal or State courts located in the County of Macomb, State of Michigan, and you hereby irrevocably submit to the exclusive personal jurisdiction of such courts for such purposes.

30. This Agreement constitutes the entire Agreement between you and the AMMN, including as to your access and use of the website and services. This Agreement supersedes and replaces all prior understandings or agreements, written or oral, regarding such subject matter.
